ATTENTION:  ST. VINCENT DE PAUL CLIENTS

The following documentation is required for individuals/ households requesting assistance from St. Vincent de Paul's Personal Service Center

For all adults (i.e.: household members 18 years old and older) you will need…

                   (1)     Picture I.D.

(2)     Social Security Card (will also accept:  Green Card, Alien I.D. Card or any Foreign I.D. Card)

(3)     Income/Benefit Verification

(4)     Expense Verification

(5)     Address Verification (lease agreement, rent receipt, utility or other bill, etc)

For all children (i.e.: household members 17 years old and younger) you will need…

(1)             Birth Certificate (or other verification of birth date and parent/guardian – i.e.:  baptismal record, school record, a current WIC folder, etc.)

(2)     Social Security Card (if available)

If you are unable to provide any of the documentation listed, St. Vincent de Paul may be unable to consider your request for assistance.
